A retrospective analysis was performed on the records of 11 patients, diagnosed with PM and fitted with both Toris K and RGPCLs in our hospital's contact lens department, who were followed up. Patient demographics, including age and gender, along with axial length, topographic keratometry readings, best-corrected visual acuity with both lens types, and subjective lens comfort assessments, were documented.
Eleven patients, each with two eyes, having an average age of 209111 years, participated in the study involving the 22 eyes. The mean AL in the right eye measured 160101 mm, while the left eye's mean AL was 15902 mm. The average values for K1 and K2, in D, were 48622 and 49422, respectively. Though RGPCLs appear beneficial for vision rehabilitation procedures, patients experience greater comfort with Toric K lenses, leading to their preference.
There is a pronounced difference in the steepness of corneal surfaces between patients with PMs and the normal population. Accordingly, the rehabilitation of their vision hinges on the utilization of specialized keratoconus lenses, including Toric K and RGPCLs. While vision rehabilitation may be more favorable with RGPCLs, the preference for Toris K lenses stems from discomfort experienced by the patients.
Subsequent to the introduction of silicone hydrogel contact lenses, many silicone-hydrogel materials have been formulated, including water-gradient lenses with a silicone hydrogel nucleus and a thin hydrogel outer membrane (like delefilcon A, verofilcon A, and lehfilcon A). Investigations into the properties of these substances have encompassed both their chemical-physical attributes and comfort aspects, but a coherent picture does not always emerge from these diverse studies. A clinicopathologic assessment was performed on placentas from our institution that were exposed to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). We located expectant mothers diagnosed with the SARS-CoV-2 virus, a timeframe from March to October 2020. The clinical data examined comprised the gestational age at delivery and the gestational age at diagnosis, in addition to maternal symptoms. Post-LASIK cataract patients with multifocal, extended depth of focus (EDOF), and monofocal intraocular lenses (IOLs) are compared and contrasted regarding functional visual outcomes and patient satisfaction levels.
Three cohorts of post-LASIK eyes, with variations in intraocular lens type (multifocal, EDOF, or monofocal), were subject to analysis. Comparing the objective preoperative and postoperative clinical metrics, such as higher-order aberrations, contrast sensitivity, and visual acuities, was paired with gathering subjective feedback from patients through questionnaires focusing on satisfaction, spectacle needs, and task capabilities. Variables were analyzed against the measure of overall patient satisfaction to find the factors correlating with satisfaction.
Ninety-seven percent of patients voiced their satisfaction, categorized as either very satisfied or satisfied. The degree of satisfaction was considerably higher with multifocal (868%, 33 of 38) and EDOF (727%, 8 of 11) IOLs in comparison to monofocal (333%, 6 of 18) IOLs. While monofocal IOLs were outperformed by EDOF IOLs, this difference was statistically significant in the intermediate category (P = 0.004). High satisfaction levels were reported for multifocal IOLs in post-LASIK patients, regardless of higher-order aberrations and reduced contrast sensitivity; regression analysis highlighted the decisive influence of uncorrected near visual function on the reported satisfaction; unexpectedly, dysphotopsias did not substantially correlate with satisfaction; therefore, multifocal IOLs provide a reasonable option for cataract surgery in patients who have had LASIK.
Although higher-order aberrations and lower contrast sensitivity were observed, multifocal lenses generated high levels of satisfaction in post-LASIK patients. Regression analysis demonstrated that uncorrected near visual function was strongly linked to the satisfaction. Dysphotopsias had a negligible impact on satisfaction scores. Multifocal IOLs represent a viable option for treating cataracts in patients with a prior LASIK history.
Prolonged lifespans and improved survival have led to a substantial increase in the number of individuals grappling with multimorbidity, raising concerns about the complexities of polypharmacy, the strain of treatment regimens, competing therapeutic objectives, and inadequate healthcare coordination. Self-management programs are becoming indispensable components of interventions striving to produce positive outcomes in this population. Yet, a detailed look at the efficacy of interventions supporting self-care in individuals with various concurrent illnesses is nonexistent. This scoping review's aim was to chart the literature related to patient-centered interventions for those managing multiple health conditions. We scrutinized multiple databases, clinical registries, and the grey literature for RCTs published between 1990 and 2019, detailing interventions that promoted self-management in individuals with multiple coexisting conditions. Incorporating 72 studies, we identified considerable heterogeneity among the participant groups, intervention methods, components, and supporting factors. The results showed that cognitive behavioral therapy served as a crucial basis for interventions, along with the integration of behavior change theories and disease management frameworks. The most prevalent coded behavioral changes were largely derived from the categories of Social Support, Feedback and Monitoring, and Goals and Planning. To ensure the successful implementation of interventions in clinical care, it is imperative to enhance the reporting of intervention methods in randomized controlled trials.
