This multicenter retrospective study aimed to assess the clinical impact and economic feasibility of using bone substitutes with volar locking plate fixation for unstable distal radial fractures in the elderly population. A database (TRON) provided the data for 1980 patients, aged 65 and above, who had DRF surgery using a VLP from 2015 to 2019. To ascertain the frequency and molecular profiles of extended-spectrum beta-lactamase-producing Gram-negative bacilli from a pediatric patient group in Gaza's hospital system.
In Gaza, four pediatric referral hospitals—Al-Nasr, Al-Rantisi, Al-Durra, and Beit Hanoun—contributed a total of 322 Gram-negative bacilli isolates for collection. The isolates were examined for ESBL production by implementing the double disk synergy method in conjunction with the CHROMagar phenotypic procedure. Employing PCR techniques targeted at the CTX-M, TEM, and SHV genes, molecular characterization of the ESBL-producing bacterial isolates was carried out. The Clinical and Laboratory Standards Institute's standardized Kirby-Bauer method was utilized to establish the antibiotic susceptibility profile.
In a phenotypic analysis of 322 tested isolates, 166 were found to be ESBL positive, accounting for 51.6 percent of the total. A comparative analysis of ESBL production in Al-Nasr, Al-Rantisi, Al-Durra, and Beit Hanoun hospitals revealed rates of 54%, 525%, 455%, and 528%, respectively. The respective prevalences of ESBL production among Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa, Acinetobacter spp., Proteus mirabilis, Enterobacter spp., Citrobacter spp., and Serratia marcescens are 553%, 634%, 178%, 571%, 333%, 285%, 384%, and 4%. Significant differences were observed in ESBL production across various samples, with urine exhibiting a 533% increase, pus 552%, blood 474%, cerebrospinal fluid (CSF) 333%, and sputum a comparatively low 25% increase. From the 322 isolates identified, 144 were subsequently screened to determine the production levels of CTX-M, TEM, and SHV. PCR analysis indicated that 85 samples (59% of the cohort examined) exhibited a minimum of one gene. The CTX-M, TEM, and SHV genes exhibited respective prevalence rates of 60%, 576%, and 383%. Meropenem and amikacin exhibited the highest susceptibility rates against ESBL-producing bacteria, with 831% and 825% respectively, while amoxicillin and cephalexin demonstrated the lowest effectiveness, achieving only 31% and 139% respectively. Significantly, ESBL-producing organisms exhibited a strong resistance to cefotaxime, ceftriaxone, and ceftazidime, demonstrating resistance rates of 795%, 789%, and 795%, respectively.
